FILLING A MEDICAL ALLOWANCE (MA) FORM

PURPOSE

This form is used by an Individual or Company that requires foreign exchange to pay for medical bills outside Nigeria.

PROCESS

1. Applicants login into the applicant portal and click on Medical Allowance on the Trade service Tab. See Figure below

2. Applicants must fill all the required fields properly before moving to the next stage.
3. Applicants must fill all the required fields properly before moving to the next stage.

1. Select Valid for FOREX as YES
2. Fill out Bank Details
3. Attach Documents

           4. After Applicant fills all the fields he/she submits a Form.

USE CASE SCENARIO

• An individual/Company wants to pay for Medical bills/services in London.
• The individual logs into the TRMS portal and clicks on Form A to start an application.
• Individual clicks on Medical Allowance.
• The individual input all the required information. See figure below

• Once submitted the individual receives an email notifying him/her that their form has been submitted.
• The bank processes the application or rejects it.
